Description

This course equips learners with the knowledge and skills required to clean and sterilise operating theatre central supply items in accordance with South African health regulations and international best practices. Participants will gain competence in infection prevention, decontamination processes, and the safe handling of sterile supplies to ensure patient safety and compliance with SAQA unit standard 110440.

  • Apply principles of infection control and aseptic techniques during the cleaning and sterilisation of operating theatre central supply items.
  • Demonstrate correct procedures for decontaminating, disinfecting, and sterilising instruments using appropriate equipment and methods.
  • Implement quality assurance checks to verify sterilisation efficacy, including biological and chemical indicators.
  • Evaluate the condition of sterile supplies and identify defects or contamination risks during storage and handling.
  • Design a workflow for the safe collection, cleaning, packaging, and distribution of central supply items in an operating theatre setting.

Course Outline

Day 1: Introduction to CSSD and Infection Control Principles

Objectives:
• Understand the role of the CSSD in infection prevention and control.
• Identify key terms and definitions related to cleaning, disinfection, and sterilisation.
• Recognise the chain of infection and how CSSD processes break it.
• Describe the legal and regulatory framework governing CSSD operations in South Africa.

Topics:
• Overview of the operating theatre and CSSD workflow.
• Infection control principles: chain of infection, standard precautions.
• Key definitions: cleaning, disinfection, sterilisation, asepsis.
• Relevant SAQA unit standards and health regulations (e.g., OHS Act, SANS standards).
• Roles and responsibilities of CSSD personnel.
• Introduction to personal protective equipment (PPE) and hand hygiene.

Day 2: Cleaning and Decontamination of Instruments and Items

Objectives:
• Describe the steps for manual and mechanical cleaning of surgical instruments.
• Identify appropriate cleaning agents and their correct use.
• Demonstrate proper sorting and preparation of items for cleaning.
• Explain the importance of decontamination before sterilisation.

Topics:
• Manual cleaning techniques: scrubbing, brushing, ultrasonic cleaning.
• Mechanical cleaning: washer-disinfectors, automated cleaning cycles.
• Cleaning agents: detergents, enzymes, disinfectants – selection and dilution.
• Handling of sharp instruments and delicate items.
• Inspection for cleanliness and damage after cleaning.
• Packaging and transport of soiled items to the CSSD.

Day 3: Disinfection and Sterilisation Methods

Objectives:
• Differentiate between various levels of disinfection and sterilisation.
• Describe the principles and operation of autoclaves (steam sterilisation).
• Explain alternative sterilisation methods: ethylene oxide, hydrogen peroxide plasma.
• Identify factors affecting sterility assurance: time, temperature, pressure, contact.

Topics:
• Levels of disinfection: low, intermediate, high; sporicidal agents.
• Steam sterilisation: autoclave types, cycles, and parameters.
• Low-temperature sterilisation: EtO, H2O2 plasma, ozone.
• Chemical sterilants and high-level disinfectants.
• Monitoring sterilisation: physical, chemical, biological indicators.
• Sterilisation of textiles, wraps, and packaging materials.

Day 4: Packaging, Storage, and Distribution of Sterile Items

Objectives:
• Select appropriate packaging materials for different items.
• Apply correct wrapping and sealing techniques to maintain sterility.
• Understand storage conditions that preserve sterility.
• Describe procedures for distributing sterile supplies to the operating theatre.

Topics:
• Packaging materials: paper, plastic, non-woven, containers; criteria for selection.
• Wrapping techniques: peel pouches, double wrapping, rigid containers.
• Labeling: contents, sterilisation date, lot number, expiry.
• Storage: shelf life, clean environment, temperature, humidity control.
• Distribution: cart management, first-expiry-first-out (FEFO) system.
• Handling of sterile items: inspection before use, avoiding contamination.

Day 5: Quality Assurance, Troubleshooting, and Practical Integration

Objectives:
• Implement quality assurance measures in CSSD processes.
• Identify common errors in cleaning, sterilisation, and packaging.
• Perform routine maintenance and troubleshooting of sterilisation equipment.
• Integrate all skills in a simulated CSSD workflow.

Topics:
• Quality assurance: audits, documentation, traceability.
• Troubleshooting: sterilisation failures, wet packs, chemical indicator issues.
• Routine equipment checks: autoclave door seals, thermometers, timers.
• Corrective actions: reprocessing, recall procedures.
• Personal and team safety: ergonomics, hazard communication.
• Overview of career pathways and professional development.

Overview

Hands-on practical sessions are essential for learners to develop competency in cleaning, sterilisation, packaging, and quality assurance procedures. These sessions simulate real-world CSSD tasks and allow direct application of theoretical knowledge under supervision.

Practical Activities
  • Manual and Mechanical Cleaning of Instruments — Learners manually clean surgical instruments using detergents and ultrasonic cleaner, then operate a washer-disinfector and inspect instruments for cleanliness. (4h)
  • Sterilisation Cycle Operation and Monitoring — Learners load an autoclave, select appropriate cycle, and run a sterilisation cycle. They place chemical and biological indicators and interpret results. (4h)
  • Packaging and Wrapping of Sterile Items — Learners practice wrapping instruments using peel pouches and double wrapping techniques, label packages, and inspect seals. (4h)
  • Quality Assurance and Troubleshooting Scenarios — Learners participate in simulated scenarios: identifying wet packs, failed biological indicators, and packaging errors. They document findings and propose corrective actions. (4h)

Summatives

Each delegate is assessed continuously throughout the course via daily exercises, scored practical assignments, and a final summative test at the end.

Practical Assignments — 30%

Practical assignments are observed and scored against a rubric during the practical sessions. Each delegate's practical mark is averaged into a single 100% score and contributes 30% to the final total.

Daily Exercises — 20%

Every training day ends with a multiple-choice exercise scored out of 100%. The scores from each daily exercise are averaged across the duration of the course to produce a Daily Average mark, which contributes 20% to the final total.

Final Test — 50%

On the last day a final summative test is written. It is a multiple-choice paper with multiple-answer questions: each question may have more than one correct option, and a single wrong selection on a question marks the entire question wrong — no partial credit. The final test is scored out of 100% and contributes 50% to the overall mark.

Certificate

Certificate of Completion

Awarded to delegates who achieve an overall mark of 50% or higher on the Final Total (Practicals 30% + Daily Average 20% + Final Test 50%).
