Course Outline

Day 1: Understanding Long-Term Illness and the Nursing Role

Objectives:
• Define long-term illness and its impact on individuals and families.
• Explain the role of the nurse in long-term care settings.
• Identify ethical and legal considerations in chronic care.
• Discuss the principles of holistic, patient-centred care.

• Overview of long-term illness: definitions, prevalence, and types.
• Biopsychosocial impact of chronic disease.
• The nursing process: assessment, diagnosis, planning, implementation, evaluation.
• Ethical principles: autonomy, beneficence, non-maleficence, justice.
• Legal frameworks: patient rights, consent, and confidentiality.
• Multidisciplinary team approach.
• Cultural sensitivity and patient-centred care.
• Documentation and reporting standards.

Day 2: Assessment and Care Planning for Chronic Conditions

Objectives:
• Conduct comprehensive health assessments for individuals with long-term illness.
• Develop individualised care plans based on assessment findings.
• Prioritise nursing interventions for common chronic conditions.
• Apply evidence-based guidelines for diabetes, hypertension, and COPD.

• Comprehensive health history and physical assessment.
• Functional assessment: activities of daily living (ADLs) and instrumental ADLs.
• Pain assessment and management.
• Nutritional and psychosocial assessment.
• Care plan development: SMART goals.
• Evidence-based care for diabetes mellitus.
• Evidence-based care for hypertension and cardiovascular disease.
• Evidence-based care for chronic respiratory conditions (COPD, asthma).

Day 3: Symptom Management and Palliative Care

Objectives:
• Identify common symptoms in long-term illness and appropriate interventions.
• Apply principles of palliative care to improve quality of life.
• Manage pain effectively using pharmacological and non-pharmacological methods.
• Support patients and families in end-of-life decision-making.

Topics:
• Symptom management: fatigue, dyspnoea, nausea, anorexia.
• Pain management: WHO analgesic ladder, non-pharmacologic interventions.
• Palliative care principles: holistic, interdisciplinary, family-centred.
• Communication with patients and families about prognosis.
• Advance care planning and living wills.
• Grief and bereavement support.
• Ethical dilemmas in end-of-life care.
• Cultural considerations in death and dying.

Day 4: Psychosocial Support and Patient Education

Objectives:
• Assess psychosocial needs of patients with chronic illness.
• Provide emotional support and coping strategies.
• Educate patients and families on self-management and lifestyle modifications.
• Promote adherence to treatment regimens.

Topics:
• Psychological impacts: depression, anxiety, adjustment disorders.
• Social impacts: isolation, financial strain, role changes.
• Motivational interviewing and behaviour change.
• Self-management education: medication adherence, diet, exercise.
• Teaching patients about monitoring: blood glucose, blood pressure.
• Family involvement and caregiver support.

Day 5: Integration, Quality Improvement, and Professional Development

• Synthesise knowledge to manage complex cases.
• Evaluate care outcomes and implement quality improvement.
• Reflect on professional practice and identify learning needs.
• Prepare for unit standard assessment.

Topics:
• Case studies: multi-morbidity and complex care coordination.
• Quality improvement: audit, feedback, and evidence-based practice.
• Interprofessional collaboration and referral pathways.
• Self-care for nurses: preventing burnout and compassion fatigue.
• Reflective practice and continuous professional development.
• Review of unit standard 120169 outcomes.
• Mock assessment and feedback.
• Action planning for clinical application.

Practicals

This course is made up of exercises and case studies delivered alongside the theory — no separate practical sessions are required to complete it.

Summatives

Each delegate is assessed continuously throughout the course and a final summative test at the end.

Daily Exercises — 30%

Every training day ends with a multiple-choice exercise scored out of 100%. The scores from each daily exercise are averaged across the duration of the course to produce a Daily Average mark, which contributes 30% to the final total.

Final Test — 70%

On the last day a final summative test is written. It is a multiple-choice paper with multiple-answer questions: each question may have more than one correct option, and a single wrong selection on a question marks the entire question wrong — no partial credit. The final test is scored out of 100% and contributes 70% to the overall mark.

Certificate

Certificate of Completion

Awarded to delegates who achieve an overall mark of 50% or higher on the Final Total (Daily Average 30% + Final Test 70%).
