Description

This course equips learners with the advanced knowledge and practical skills required to provide primary emergency care and first aid as an advanced first responder. It covers the assessment and management of life-threatening emergencies, including trauma, medical emergencies, and environmental incidents, in accordance with South African protocols and regulations. The course prepares learners to take charge at an emergency scene, ensuring effective and safe care until professional medical help arrives.

Learning Outcomes

  • Demonstrate the ability to assess an emergency scene and ensure safety for the responder, patient, and bystanders.
  • Apply advanced first aid techniques to manage life-threatening conditions, including airway obstruction, severe bleeding, shock, and cardiac arrest.
  • Evaluate and manage medical emergencies such as asthma, epilepsy, diabetes, and allergic reactions.
  • Implement appropriate treatment for trauma injuries, including fractures, dislocations, burns, and head or spinal injuries.
  • Analyze environmental emergencies, such as hypothermia, heatstroke, and poisoning, and provide effective care.
  • Demonstrate effective communication and teamwork during emergency response, including handover to advanced medical care.

Target Audience

This course is designed for individuals who are designated as advanced first responders in the workplace or community, including safety officers, security personnel, and employees in high-risk industries. It is also suitable for those seeking to upgrade their first aid qualifications to an advanced level.

Prerequisites

None — open enrollment, although prior completion of a basic first aid course is recommended.

Course Outline

Day 1: Foundations of Advanced Emergency Care

Objectives:
• Understand the scope and legal framework of advanced first response in South Africa.
• Identify the roles and responsibilities of an advanced first responder.
• Apply principles of scene safety and situational awareness.
• Demonstrate effective communication and documentation skills.

Topics:
• Introduction to advanced first responder scope and SAQA standards.
• Legal and ethical considerations (Good Samaritan Act, consent, confidentiality).
• Scene assessment and safety protocols.
• Communication strategies with patients, bystanders, and emergency services.
• Documentation and record-keeping.
• Infection control and personal protective equipment.

Day 2: Advanced Patient Assessment and Airway Management

Objectives:
• Perform a comprehensive patient assessment using the ABCDE approach.
• Identify and manage life-threatening airway and breathing emergencies.
• Apply advanced airway adjuncts and suctioning techniques.
• Recognize signs of respiratory failure and distress.

Topics:
• Primary and secondary survey techniques.
• Vital signs monitoring and interpretation.
• Airway management: oropharyngeal and nasopharyngeal airways, suctioning.
• Oxygen therapy delivery systems.
• Basic and advanced breathing assessment.
• Management of respiratory emergencies (asthma, anaphylaxis, pneumothorax).

Day 3: Circulation, Shock, and Trauma Care

Objectives:
• Assess and manage circulatory emergencies including shock and hemorrhage.
• Apply advanced bleeding control techniques.
• Perform splinting and immobilization for musculoskeletal injuries.
• Manage traumatic injuries to the head, spine, chest, and abdomen.

Topics:
• Shock recognition and management (hypovolemic, cardiogenic, distributive).
• Hemorrhage control: direct pressure, tourniquets, haemostatic dressings.
• Fracture management and splinting.
• Spinal immobilization and log roll.
• Chest trauma: flail chest, tension pneumothorax, open pneumothorax.
• Abdominal and pelvic injuries.

Day 4: Medical Emergencies and Environmental Injuries

Objectives:
• Recognize and manage common medical emergencies (cardiac, diabetic, neurological).
• Administer basic medications and assist with prescribed medications.
• Manage environmental emergencies (heat, cold, drowning, envenomation).
• Apply triage principles in multiple casualty incidents.

Topics:
• Cardiac emergencies: chest pain, CPR, AED use.
• Diabetic emergencies: hypoglycemia and hyperglycemia.
• Neurological emergencies: stroke, seizures, altered mental status.
• Medication administration: aspirin, oxygen, glucose, epinephrine auto-injectors.
• Environmental emergencies: hypothermia, heat stroke, snake bites, drowning.
• Triage systems (START, JumpSTART) and MCI management.

Day 5: Special Populations, Transport, and Practical Integration

Objectives:
• Adapt emergency care for special populations (paediatrics, geriatrics, pregnancy).
• Understand principles of safe patient transport and handover.
• Integrate all skills in simulated emergency scenarios.
• Demonstrate competency in advanced first responder skills.

Topics:
• Paediatric emergencies: assessment, airway, dehydration, fever.
• Geriatric considerations: polypharmacy, falls, dementia.
• Obstetric emergencies: childbirth, postpartum hemorrhage.
• Patient transport: stretcher operations, ambulance safety.
• Handover procedures (SBAR).
• Scenario-based integration and skills assessment.

Practicals

Overview

Practicals form a critical component of this course, allowing learners to apply advanced skills in realistic scenarios. Supervised practice ensures proficiency in airway management, bleeding control, splinting, and patient assessment, culminating in integrated emergency simulations.

Practical Activities
  • Airway Management and Ventilation — Learners practice inserting oropharyngeal and nasopharyngeal airways, suctioning, and using bag-valve-mask ventilation on manikins. (4h)
  • Hemorrhage Control and Shock Management — Learners apply direct pressure, tourniquets, and haemostatic dressings on simulated wounds, and manage shock scenarios. (4h)
  • Splinting and Spinal Immobilization — Learners practice splinting long bone fractures and performing spinal immobilization including cervical collar application and log roll. (4h)
  • Integrated Emergency Scenarios — Learners respond to simulated multi-casualty incidents and complex emergencies integrating all skills learned. (4h)

Summatives

Each delegate is assessed continuously throughout the course via daily exercises, scored practical assignments, and a final summative test at the end.

Practical Assignments — 30%

Practical assignments are observed and scored against a rubric during the practical sessions. Each delegate's practical mark is averaged into a single 100% score and contributes 30% to the final total.

Daily Exercises — 20%

Every training day ends with a multiple-choice exercise scored out of 100%. The scores from each daily exercise are averaged across the duration of the course to produce a Daily Average mark, which contributes 20% to the final total.

Final Test — 50%

On the last day a final summative test is written. It is a multiple-choice paper with multiple-answer questions: each question may have more than one correct option, and a single wrong selection on a question marks the entire question wrong — no partial credit. The final test is scored out of 100% and contributes 50% to the overall mark.

Certificate of Completion

Awarded to delegates who achieve an overall mark of 50% or higher on the Final Total (Practicals 30% + Daily Average 20% + Final Test 50%).
