This course equips healthcare and social service professionals with the knowledge and skills to effectively support clients with psychiatric disorders, promoting optimal functioning and improved quality of life. Course Outline

Day 1: Introduction to Psychiatric Disorders and Support Frameworks

Objectives:
• Define psychiatric disorders and their prevalence in South Africa
• Understand the biopsychosocial model of mental health
• Identify the role of support workers in promoting optimal functioning
• Outline ethical and legal considerations in mental health support
• Describe the impact of stigma on clients with psychiatric disorders

Topics:
• Overview of common psychiatric disorders (depression, anxiety, bipolar, schizophrenia)
• Biopsychosocial model and its application
• Scope of practice for support workers
• Mental Health Care Act (South Africa) and client rights
• Stigma, discrimination, and cultural considerations

Day 2: Communication and Therapeutic Rapport

Objectives:
• Apply active listening and empathy in client interactions
• Use person-centered language and avoid labelling
• Identify barriers to effective communication
• Develop skills for de-escalating emotionally charged situations
• Understand the importance of non-verbal communication

Topics:
• Active listening techniques
• Empathic responding and validation
• Communication barriers (e.g., cognitive distortions, sensory issues)
• De-escalation strategies
• Cultural sensitivity in communication

Day 3: Understanding Depression and Anxiety Disorders

Objectives:
• Describe symptoms and subtypes of depression and anxiety
• Explain how these disorders affect daily functioning
• Identify supportive interventions for mood and anxiety symptoms
• Recognise signs of suicidal ideation and crisis response
• Apply self-care strategies to prevent burnout

Topics:
• Major depressive disorder, dysthymia, and bipolar depression
• Generalized anxiety, panic, social anxiety, and PTSD
• Impact on motivation, sleep, and social interaction
• Supportive techniques: behavioural activation, grounding exercises
• Suicide risk assessment and emergency protocols

Day 4: Supporting Clients with Psychotic Disorders

Objectives:
• Describe schizophrenia spectrum and other psychotic disorders
• Understand positive and negative symptoms
• Develop strategies for managing delusions and hallucinations
• Promote medication adherence and side-effect management
• Foster community integration and reduce relapse risk

Topics:
• Schizophrenia, schizoaffective disorder, brief psychotic disorder
• Positive symptoms: hallucinations, delusions, disorganised speech
• Negative symptoms: anhedonia, avolition, social withdrawal
• Psychosocial interventions: CBTp, social skills training
• Role of family support and psychoeducation

Day 5: Personality Disorders and Behavioural Support

Objectives:
• Identify cluster A, B, and C personality disorders
• Understand challenges in supporting clients with borderline personality disorder
• Apply dialectical behaviour therapy (DBT) skills in daily support
• Set boundaries and manage manipulative behaviours
• Promote emotional regulation and interpersonal effectiveness

Topics:
• Overview of personality disorders (paranoid, borderline, antisocial, avoidant)
• Core features of borderline personality disorder
• DBT skills: mindfulness, distress tolerance, emotion regulation
• Boundary setting and consistency
• Crisis intervention for self-harm and suicidal behaviour

Day 6: Substance Use Disorders and Dual Diagnosis

Objectives:
• Describe substance use disorders and withdrawal syndromes
• Understand the link between substance use and psychiatric disorders
• Apply motivational interviewing to encourage change
• Develop relapse prevention plans
• Coordinate care with addiction services

Topics:
• Common substances: alcohol, cannabis, methamphetamine, opioids
• Dual diagnosis: co-occurring mental illness and substance use
• Stages of change and motivational interviewing
• Harm reduction vs. abstinence approaches
• Referral pathways to rehabilitation centres

Day 7: Trauma-Informed Care and PTSD

Objectives:
• Define trauma and its neurobiological effects
• Understand complex trauma and its long-term impact
• Apply trauma-informed principles (safety, trustworthiness, choice)
• Support clients with PTSD symptoms
• Avoid re-traumatisation in care settings

Topics:
• Types of trauma: acute, chronic, complex
• Neurobiology of trauma: HPA axis, amygdala, hippocampus
• Trauma-informed care principles
• PTSD symptoms: intrusive memories, avoidance, hyperarousal
• Grounding techniques and stabilisation

Day 8: Cognitive Impairments and Neurodevelopmental Disorders

Objectives:
• Describe intellectual disability, autism spectrum disorder, and ADHD
• Understand how cognitive impairments affect daily functioning
• Adapt communication and support strategies
• Promote independence and skill development
• Address challenging behaviours with positive behaviour support

Topics:
• Intellectual disability: levels and support needs
• Autism spectrum: sensory sensitivities, social communication
• ADHD: inattention, hyperactivity, executive dysfunction
• Positive behaviour support plans
• Environmental modifications and visual aids

Day 9: Medication Management and Side Effects

Objectives:
• Identify common psychotropic medications and their classes
• Understand side effects and their impact on functioning
• Support medication adherence and monitor effects
• Recognise signs of adverse reactions and overdose
• Liaise with prescribing professionals

Topics:
• Antidepressants, antipsychotics, mood stabilisers, anxiolytics
• Common side effects: weight gain, sedation, extrapyramidal symptoms
• Strategies to improve adherence: routines, reminders, education
• Monitoring for tardive dyskinesia, serotonin syndrome
• Documentation and reporting to healthcare team

Day 10: Crisis Intervention and Safety Planning

Objectives:
• Identify signs of psychiatric crisis (psychosis, mania, severe depression)
• Apply crisis de-escalation techniques
• Develop a safety plan with the client
• Understand involuntary admission procedures
• Implement self-care after critical incidents

• Crisis phases: escalation, crisis, post-crisis
• Verbal de-escalation: calm tone, open questions, validation
• Safety planning: triggers, coping strategies, support contacts
• Mental Health Care Act: 72-hour assessment, court orders
• Debriefing and staff support

Day 11: Psychosocial Rehabilitation and Community Integration

• Support clients in developing independent living skills
• Facilitate social inclusion and meaningful activities
• Collaborate with community resources and families
• Promote recovery-oriented practice

Day 12: Advocacy, Rights, and Ethical Dilemmas

• Identify ethical dilemmas (confidentiality, autonomy, duty of care)
• Apply ethical decision-making frameworks
• Promote client rights and informed consent
• Address discrimination and access barriers

• Case studies: balancing confidentiality and safety
• South African Human Rights Commission and Mental Health Review Boards

Day 13: Integration, Self-Care, and Professional Development

Objectives:
• Integrate knowledge across all days into a comprehensive support plan
• Develop a personal self-care plan to prevent burnout
• Identify continuing professional development opportunities
• Evaluate own learning and areas for growth
• Present a case study demonstrating holistic support

• Comprehensive case study: assessment, intervention, evaluation
• Self-care strategies: supervision, mindfulness, boundaries
• Professional development: workshops, reading, peer networks
• Reflective practice and journaling
• Course evaluation and certification

This course is made up of exercises and case studies delivered alongside the theory — no separate practical sessions are required to complete it.

Summatives

Each delegate is assessed continuously throughout the course and a final summative test at the end.

Daily Exercises — 30%

Every training day ends with a multiple-choice exercise scored out of 100%. The scores from each daily exercise are averaged across the duration of the course to produce a Daily Average mark, which contributes 30% to the final total.

Final Test — 70%

On the last day a final summative test is written. It is a multiple-choice paper with multiple-answer questions: each question may have more than one correct option, and a single wrong selection on a question marks the entire question wrong — no partial credit. The final test is scored out of 100% and contributes 70% to the overall mark.

Certificate of Completion

Awarded to delegates who achieve an overall mark of 50% or higher on the Final Total (Daily Average 30% + Final Test 70%).
