Course Outline

Day 1: Fundamentals of Fault Report Analysis

Objectives:
• Understand the structure and components of a fault report
• Identify common fault types and their classifications
• Apply basic analysis techniques to interpret fault data
• Recognise the role of fault reports in decision-making processes
• Develop skills to extract relevant information from fault reports

Topics:
• Introduction to fault reports: purpose and key elements
• Classification of faults: mechanical, electrical, software, human error
• Data extraction methods: reading and interpreting fault codes and logs
• Basic analysis techniques: trend identification and root cause analysis
• Linking fault reports to operational procedures and standards
• Practical exercise: analysing sample fault reports

Day 2: Decision Making Procedures and Practical Application

Objectives:
• Apply decision-making frameworks to fault report findings
• Evaluate corrective actions based on analysis outcomes
• Prioritise faults using risk assessment and impact analysis
• Document decisions and communicate recommendations effectively
• Integrate analysis and decision-making into maintenance workflows

Topics:
• Decision-making models: cost-benefit analysis, risk matrix, and urgency assessment
• Selecting appropriate corrective actions: repair, replace, monitor, or escalate
• Prioritisation techniques: criticality and impact on operations
• Documentation standards for decisions and follow-up actions
• Communication strategies for presenting analysis and recommendations
• Case study: end-to-end fault report analysis and decision making

Overview

Practical sessions allow learners to apply fault report analysis and decision-making procedures in simulated scenarios. Learners will work with authentic fault reports, perform root cause analysis, and make justified decisions using provided frameworks.

Practical Activities
  • Fault Report Data Extraction and Classification — Learners receive sample fault reports and must extract key data, classify faults, and identify trends. (2h)
  • Root Cause Analysis Workshop — Using fault scenarios, learners apply root cause analysis techniques (e.g., 5 Whys, fishbone diagram) to determine underlying causes. (2h)
  • Decision Making Simulation — Learners are presented with analysed fault reports and must select corrective actions, prioritise, and justify decisions using risk and cost-benefit assessments. (2h)

Summatives

Each delegate is assessed continuously throughout the course via daily exercises, scored practical assignments, and a final summative test at the end.

Practical Assignments — 30%

Practical assignments are observed and scored against a rubric during the practical sessions. Each delegate's practical mark is averaged into a single 100% score and contributes 30% to the final total.

Daily Exercises — 20%

Every training day ends with a multiple-choice exercise scored out of 100%. The scores from each daily exercise are averaged across the duration of the course to produce a Daily Average mark, which contributes 20% to the final total.

Final Test — 50%

On the last day a final summative test is written. It is a multiple-choice paper with multiple-answer questions: each question may have more than one correct option, and a single wrong selection on a question marks the entire question wrong — no partial credit. The final test is scored out of 100% and contributes 50% to the overall mark.

Certificate

Certificate of Completion

Awarded to delegates who achieve an overall mark of 50% or higher on the Final Total (Practicals 30% + Daily Average 20% + Final Test 50%).
