Description

This course equips laboratory technicians and medical professionals with the theoretical knowledge and practical skills required to examine urine and sputum specimens under a microscope. Participants will learn to identify key microscopic elements such as cells, casts, crystals, and microorganisms, enabling accurate diagnosis and monitoring of patient conditions.

Course Outline

Day 1: Introduction to Microscopic Examination of Urine and Sputum

Objectives:
• Understand the role of microscopic examination in clinical diagnosis
• Identify the components of a light microscope and their functions
• Describe the principles of specimen collection and handling
• Recognise safety precautions and infection control measures
• Outline the regulatory framework and quality assurance standards

Topics:
• Overview of urinalysis and sputum analysis
• Microscopy: parts, care, and maintenance
• Specimen collection: urine (midstream, catheter) and sputum (expectorated, induced)
• Transport, storage, and rejection criteria
• Safety: PPE, biohazard waste disposal, spill management
• Introduction to SAQA unit standard 252398
• Quality control and documentation

Day 2: Urine Specimen Preparation and Microscopic Examination

Objectives:
• Perform standardised urine specimen preparation techniques
• Distinguish between different types of urine sediment preparations
• Apply correct microscopy techniques for urine examination
• Identify normal and abnormal cellular elements in urine
• Interpret findings in the context of common urinary tract conditions

Topics:
• Urine specimen processing: centrifugation, decanting, resuspension
• Preparation of wet mount and stained slides
• Microscopic examination: low-power and high-power fields
• Identification of red blood cells, white blood cells, epithelial cells
• Casts: hyaline, granular, cellular, waxy, fatty
• Crystals: common (calcium oxalate, uric acid) and pathological
• Reporting and grading of findings

Day 3: Advanced Urine Microscopy and Clinical Correlations

Objectives:
• Identify less common urine sediment elements (parasites, bacteria, fungi)
• Correlate microscopic findings with clinical conditions
• Perform semi-quantitative analysis and grading
• Recognise artifacts and distinguish from genuine findings
• Apply quality control measures to ensure accuracy

Topics:
• Bacteria, yeast, and parasites in urine (Trichomonas, Schistosoma)
• Lipiduria, oval fat bodies, and cholesterol crystals
• Clinical correlations: UTI, glomerulonephritis, nephrotic syndrome
• Artifacts: starch, air bubbles, debris
• Semi-quantitative methods: per high-power field (hpf) vs per low-power field (lpf)
• Internal quality control: use of control samples
• Troubleshooting common microscopy issues

Day 4: Sputum Specimen Preparation and Microscopic Examination

Objectives:
• Describe the role of sputum microscopy in respiratory diagnosis
• Perform proper sputum processing and slide preparation
• Identify cellular components in sputum
• Recognise microorganisms and other elements
• Apply grading systems for sputum quality and findings

Topics:
• Sputum examination: indications and limitations
• Macroscopic assessment: colour, consistency, blood, mucoid
• Wet mount and Gram stain preparation
• Identification of epithelial cells, neutrophils, eosinophils
• Bacteria: morphology, Gram reaction, and arrangement
• Fungi and parasites in sputum
• Sputum quality scoring (Bartlett's criteria, Geckler classification)

Day 5: Staining Techniques for Urine and Sputum

Objectives:
• Perform Gram stain and Ziehl-Neelsen stain on sputum
• Apply special stains for urine sediment (Sudan III, Prussian blue)
• Interpret stained preparations correctly
• Differentiate between acid-fast and non-acid-fast organisms
• Understand the role of stains in enhancing diagnostic accuracy

• Gram stain: principles, procedure, quality control
• Ziehl-Neelsen stain for acid-fast bacilli (AFB)
• Modified Ziehl-Neelsen for Nocardia
• Sudan III stain for fat in urine
• Prussian blue stain for hemosiderin
• Giemsa stain for intracellular organisms
• Staining artifacts and troubleshooting

Day 6: Quality Assurance, Troubleshooting, and Reporting

Objectives:
• Implement quality assurance protocols in microscopy
• Troubleshoot common errors in specimen preparation and examination
• Generate accurate and clear reports
• Apply critical thinking to resolve discrepancies
• Understand proficiency testing and external quality assessment

• Quality assurance: internal controls, equipment checks, reagent QC
• Common errors: improper centrifugation, staining time, microscope calibration
• Reporting formats: descriptive, semi-quantitative, and grading
• Critical value reporting and communication
• Proficiency testing programs

Day 7: Integration, Case Studies, and Competency Assessment

Overview

Hands-on practical sessions are essential for developing competence in microscopic examination of urine and sputum. Learners will practice specimen preparation, microscopy, staining, and reporting under supervision, ensuring they can apply theoretical knowledge in a laboratory setting.

Practical Activities
  • Microscope Setup and Maintenance — Learners practice setting up, focusing, and cleaning a light microscope, and perform basic maintenance checks. (2h)
  • Urine Specimen Preparation and Microscopy — Learners prepare urine specimens (centrifugation, decanting, resuspension) and examine wet mounts under low and high power, identifying formed elements. (6h)
  • Sputum Processing and Gram Stain — Learners process sputum specimens, prepare smears, perform Gram stain, and examine for cellular content and bacteria. (6h)
  • Ziehl-Neelsen Stain for AFB — Learners perform Ziehl-Neelsen staining on sputum smears and examine for acid-fast bacilli. (4h)
  • Integrated Practical Assessment — Learners complete a timed practical assessment covering urine and sputum preparation, staining, microscopy, and reporting, simulating real-world conditions. (6h)

Summatives

Each delegate is assessed continuously throughout the course via daily exercises, scored practical assignments, and a final summative test at the end.

Practical Assignments — 30%

Practical assignments are observed and scored against a rubric during the practical sessions. Each delegate's practical mark is averaged into a single 100% score and contributes 30% to the final total.

Daily Exercises — 20%

Every training day ends with a multiple-choice exercise scored out of 100%. The scores from each daily exercise are averaged across the duration of the course to produce a Daily Average mark, which contributes 20% to the final total.

Final Test — 50%

On the last day a final summative test is written. It is a multiple-choice paper with multiple-answer questions: each question may have more than one correct option, and a single wrong selection on a question marks the entire question wrong — no partial credit. The final test is scored out of 100% and contributes 50% to the overall mark.

Awarded to delegates who achieve an overall mark of 50% or higher on the Final Total (Practicals 30% + Daily Average 20% + Final Test 50%).
