Description

This course equips learners with the knowledge and skills to provide and implement primary health care services within community settings. It focuses on understanding the principles of primary health care, conducting community health assessments, and developing appropriate interventions to improve health outcomes.

Course Outline

Day 1: Introduction to Primary Health Care in South Africa

Objectives:
• Understand the principles and history of Primary Health Care (PHC) in South Africa.
• Identify the key components of the PHC approach.
• Explain the role of community participation in PHC.
• Describe the structure of the South African health system.

Topics:
• Definition and principles of Primary Health Care
• Historical context: Alma-Ata Declaration and South Africa's health reforms
• The PHC package in South Africa
• Levels of care: primary, secondary, tertiary
• Community-based health services and outreach
• Role of community health workers
• Introduction to the National Health Insurance (NHI) and its PHC focus

Day 2: Community Health Assessment and Needs Analysis

Objectives:
• Conduct a basic community health assessment using participatory methods.
• Identify priority health needs in a community.
• Use data to inform PHC planning.
• Understand social determinants of health.

Topics:
• Community health assessment tools (e.g., windshield surveys, interviews)
• Participatory Rural Appraisal (PRA) techniques
• Social determinants of health: poverty, education, housing, sanitation
• Common health indicators and data sources
• Prioritizing health needs: criteria and stakeholder input
• Case study: mapping a local community's health profile

Day 3: Maternal, Child, and Reproductive Health in Communities

Objectives:
• Describe key interventions for maternal and child health in PHC.
• Understand family planning and reproductive health services.
• Recognize danger signs in pregnancy and newborn care.
• Promote immunization and growth monitoring.

• Antenatal care: schedule, screening, and prevention of mother-to-child transmission (PMTCT)
• Skilled birth attendance and emergency obstetric care
• Postnatal care and neonatal resuscitation
• Family planning methods and counselling
• Expanded Programme on Immunisation (EPI) schedule
• Growth monitoring and nutrition interventions
• Adolescent reproductive health

Day 4: Communicable Disease Prevention and Control

Objectives:
• Understand the epidemiology of major communicable diseases in South Africa.
• Implement infection prevention and control measures.
• Apply strategies for TB, HIV, and STI management.
• Conduct contact tracing and outbreak investigation.

• Overview of communicable diseases: TB, HIV/AIDS, STIs, malaria, cholera
• HIV testing, ART initiation, and adherence support
• TB diagnosis, DOTS strategy, and multi-drug resistant TB
• Infection prevention and control (IPC) at clinic and community level
• Contact tracing and outbreak investigation steps
• Vaccination campaigns and disease surveillance
• Role of community health workers in disease notification

Day 5: Non-Communicable Diseases and Mental Health in Primary Care

Objectives:
• Identify risk factors and screening methods for NCDs.
• Manage hypertension, diabetes, and asthma at primary care level.
• Recognize common mental health conditions and provide first-line support.
• Promote healthy lifestyles in communities.

Topics:
• Epidemiology of NCDs in South Africa: cardiovascular diseases, diabetes, cancer, chronic respiratory diseases
• Screening and diagnosis: blood pressure, blood glucose, BMI, peak flow
• Management protocols for hypertension and diabetes at PHC level
• Lifestyle interventions: diet, exercise, smoking cessation
• Common mental health conditions: depression, anxiety, substance abuse
• Mental health first aid and referral pathways
• Integration of NCD and mental health services in PHC

Day 6: Health Promotion, Education, and Community Engagement

Objectives:
• Design a health promotion campaign for a community.
• Use effective communication techniques for health education.
• Facilitate community dialogues and support groups.
• Evaluate health promotion activities.

• Principles of health promotion and the Ottawa Charter
• Behavior change models: Health Belief Model, Transtheoretical Model
• Developing health education materials for low-literacy audiences
• Communication skills: active listening, motivational interviewing
• Community engagement strategies: door-to-door, mass media, social media
• Support groups for chronic diseases and mental health
• Monitoring and evaluation of health promotion activities

Day 7: Implementation, Monitoring, and Integration of PHC Services

Objectives:
• Develop an implementation plan for a PHC program.
• Understand monitoring and evaluation frameworks.
• Integrate PHC services with other sectors (e.g., social development, education).
• Address ethical and legal considerations in community health.

Topics:
• Steps in implementing a PHC program: planning, resources, timeline
• Logistical considerations: supply chain, staffing, transport
• Monitoring and evaluation: indicators, data collection, reporting
• Intersectoral collaboration for health: water, sanitation, nutrition, education
• Ethical principles: confidentiality, consent, cultural sensitivity
• Legal framework: Patients' Rights Charter, Children's Act, Mental Health Care Act
• Final assessment: case study presentation and group feedback

This course is made up of exercises and case studies delivered alongside the theory — no separate practical sessions are required to complete it.

Summatives

Each delegate is assessed continuously throughout the course and a final summative test at the end.

Daily Exercises — 30%

Every training day ends with a multiple-choice exercise scored out of 100%. The scores from each daily exercise are averaged across the duration of the course to produce a Daily Average mark, which contributes 30% to the final total.

Final Test — 70%

On the last day a final summative test is written. It is a multiple-choice paper with multiple-answer questions: each question may have more than one correct option, and a single wrong selection on a question marks the entire question wrong — no partial credit. The final test is scored out of 100% and contributes 70% to the overall mark.

Certificate

Certificate of Completion

Awarded to delegates who achieve an overall mark of 50% or higher on the Final Total (Daily Average 30% + Final Test 70%).
