Course Outline

Day 1: Foundations of Engineering Risk Management

Objectives:
• Define key concepts: risk, hazard, likelihood, consequence, and loss control
• Explain the principles and importance of risk management in engineering contexts
• Identify relevant legislation and standards (e.g., OHS Act, ISO 31000)
• Distinguish between different types of risk: technical, operational, financial, and safety
• Outline the risk management process: identification, analysis, evaluation, treatment, monitoring

• Legal and regulatory framework (OHS Act, SANS/ISO standards)
• Types of risks in engineering environments
• Overview of the risk management process (ISO 31000)

Day 2: Risk Identification and Analysis Techniques

Objectives:
• Apply systematic methods to identify hazards and risks in engineering systems
• Use qualitative and quantitative risk analysis techniques
• Conduct a Hazard Identification and Risk Assessment (HIRA)
• Understand Failure Mode and Effects Analysis (FMEA) and its application
• Interpret risk matrices and prioritise risks for treatment

• Hazard identification methods: checklists, brainstorming, HAZOP, what-if analysis
• Risk analysis techniques: qualitative (risk matrices), semi-quantitative, quantitative (FMEA, fault tree analysis)
• Conducting a HIRA: step-by-step process
• Failure Mode and Effects Analysis (FMEA) in engineering
• Risk ranking and prioritisation using likelihood and consequence
• Practical exercise: HIRA for a typical engineering process

Day 3: Risk Evaluation and Treatment Strategies

Objectives:
• Evaluate risks against established criteria to determine acceptability
• Select appropriate risk treatment options: avoid, reduce, transfer, accept
• Design control measures using the hierarchy of controls
• Develop risk treatment plans and action items
• Understand residual risk and risk acceptance criteria

Topics:
• Risk evaluation: comparing risk levels against acceptable criteria
• Hierarchy of controls: elimination, substitution, engineering controls, administrative controls, PPE
• Risk treatment options: avoidance, reduction, transfer (insurance), acceptance
• Developing a risk treatment plan with SMART actions
• Residual risk and risk acceptance
• Cost-benefit analysis of control measures
• Case study: risk treatment in a manufacturing plant

Day 4: Loss Control and Incident Management

Objectives:
• Define loss control and its relationship to risk management
• Implement proactive loss prevention strategies
• Apply incident investigation techniques to identify root causes
• Develop corrective and preventive actions (CAPA)
• Understand emergency preparedness and response planning

Topics:
• Principles of loss control: prevention, mitigation, recovery
• Proactive monitoring: inspections, audits, safety observations
• Incident investigation: immediate, contributing, and root causes
• Root cause analysis tools: 5 Whys, fishbone diagram, fault tree
• Corrective and preventive actions (CAPA) and close-out
• Emergency response planning and drills
• Business continuity and disaster recovery basics

Day 5: Integration, Monitoring, and Continuous Improvement

Objectives:
• Establish risk management performance indicators (KPIs)
• Monitor and review risk controls and treatment plans
• Apply continuous improvement tools (Plan-Do-Check-Act)
• Communicate risk management results to stakeholders
• Develop a personal action plan for implementing risk management in the workplace

• Key performance indicators for risk management and loss control
• Monitoring techniques: trend analysis, audits, management reviews
• Continuous improvement: PDCA cycle, feedback loops
• Reporting and communication of risk information
• Integrating risk management into project lifecycles and daily operations
• Review of course and assessment preparation
• Final assessment (written test and/or practical simulation)

Overview

The practicals reinforce theoretical knowledge through hands-on exercises in risk identification, analysis, treatment planning, incident investigation, and emergency response. Learners apply techniques to real-world engineering scenarios, building competence in using risk assessment tools and implementing loss control measures.

Practical Activities
  • HIRA Practical: Hazard Identification and Risk Assessment — Learners conduct a HIRA on a simulated engineering process (e.g., chemical mixing or machine operation), identifying hazards, analysing risks, and documenting findings. (4h)
  • FMEA Workshop: Failure Mode and Effects Analysis — In teams, learners perform an FMEA on a simple engineering system (e.g., a pump or conveyor), identifying failure modes, effects, causes, and controls. (4h)
  • Incident Investigation Simulation — Learners investigate a simulated workplace incident, gather evidence, interview witnesses, and perform a root cause analysis using the 5 Whys and fishbone diagram. (4h)
  • Risk Treatment Planning and Emergency Drill — Teams create a risk treatment plan for a given scenario, then participate in an emergency drill (e.g., fire or chemical spill response) and debrief. (4h)

Summatives

Each delegate is assessed continuously throughout the course via daily exercises, scored practical assignments, and a final summative test at the end.

Practical Assignments — 30%

Practical assignments are observed and scored against a rubric during the practical sessions. Each delegate's practical mark is averaged into a single 100% score and contributes 30% to the final total.

Daily Exercises — 20%

Every training day ends with a multiple-choice exercise scored out of 100%. The scores from each daily exercise are averaged across the duration of the course to produce a Daily Average mark, which contributes 20% to the final total.

Final Test — 50%

On the last day a final summative test is written. It is a multiple-choice paper with multiple-answer questions: each question may have more than one correct option, and a single wrong selection on a question marks the entire question wrong — no partial credit. The final test is scored out of 100% and contributes 50% to the overall mark.
