Description

This course equips laboratory professionals with the knowledge and skills to implement and maintain quality practices in a medical laboratory setting. Participants will learn to apply quality assurance principles, perform quality control procedures, and contribute to continuous quality improvement, ensuring reliable and accurate laboratory results.

Course Outline

Day 1: Introduction to Quality in Medical Laboratories

Objectives:
• Define quality and its importance in medical laboratory practice
• Identify key quality standards and regulations in South Africa
• Explain the role of quality management systems (QMS)
• Describe the consequences of poor quality in laboratory testing

Topics:
• What is quality in a medical laboratory?
• Overview of ISO 15189 and SANAS requirements
• Quality management system components
• Quality control vs quality assurance
• The cost of quality and non-conformance
• Introduction to total quality management (TQM)

Day 2: Pre-Analytical Quality Processes

Objectives:
• Identify pre-analytical variables affecting test results
• Describe proper patient preparation and sample collection procedures
• Explain sample handling, transport, and storage requirements
• Apply procedures to minimise pre-analytical errors

Topics:
• Patient identification and preparation
• Specimen collection techniques (blood, urine, etc.)
• Sample labelling and requisition forms
• Transport and storage conditions
• Common pre-analytical errors and prevention
• Documentation and chain of custody

Day 3: Analytical Quality Processes

Objectives:
• Explain internal quality control (IQC) principles
• Describe external quality assessment (EQA) schemes
• Apply Westgard rules for QC interpretation
• Calibrate and maintain laboratory equipment

• IQC materials and frequency
• Westgard rules and multi-rules
• EQA programmes and proficiency testing
• Calibration procedures and traceability
• Equipment maintenance and troubleshooting
• Method validation and verification

Day 4: Post-Analytical Quality Processes

Objectives:
• Describe procedures for result review and validation
• Explain critical value reporting and communication
• Apply proper record-keeping and data management
• Identify sources of post-analytical error

• Result verification and authorisation
• Critical and panic value protocols
• Report formatting and delivery
• Laboratory information system (LIS) quality
• Document control and archiving
• Post-analytical error analysis

Day 5: Quality Audits and Corrective Actions

Objectives:
• Describe internal and external audit processes
• Conduct a basic quality audit
• Apply root cause analysis to non-conformances
• Implement corrective and preventive actions (CAPA)

Topics:
• Types of audits (internal, external, vertical, horizontal)
• Audit planning and checklists
• Root cause analysis tools (5 Whys, fishbone)
• CAPA process and documentation
• Effectiveness checks
• Audit reporting and follow-up

Day 6: Risk Management and Continuous Improvement

Objectives:
• Apply risk management principles in laboratory settings
• Use quality indicators to monitor performance
• Implement continuous improvement methodologies
• Understand the role of quality in patient safety

Topics:
• Risk assessment and FMEA
• Quality indicators and dashboards
• Plan-Do-Check-Act (PDCA) cycle
• Lean and Six Sigma in laboratories
• Incident reporting and management
• Patient safety and quality culture

Day 7: Integration and Assessment

Objectives:
• Integrate all quality aspects into a cohesive QMS
• Prepare for an accreditation audit
• Demonstrate competence through case studies
• Develop a personal quality improvement plan

• Review of QMS elements
• Mock audit exercise
• Case studies on quality failures
• Developing a quality manual
• Professional ethics and responsibility
• Final assessment and course evaluation

Practicals

24 hours of practicals To be conducted online or on-campus or in-house
Overview

Practical sessions are essential to apply theoretical quality concepts in a simulated laboratory environment. Learners will perform sample handling, QC analysis, equipment calibration, audit exercises, and corrective action implementation to develop hands-on competence.

Practical Activities
  • Specimen Collection and Handling — Learners practice proper venepuncture, sample labelling, centrifugation, and storage procedures using simulated specimens. (6h)
  • Internal Quality Control and Westgard Rules — Learners run IQC samples on a chemistry analyser, interpret QC charts using Westgard rules, and take corrective actions. (6h)
  • Equipment Calibration and Maintenance — Learners calibrate a pipette, perform daily maintenance on a spectrophotometer, and record results. (4h)
  • Internal Audit Exercise — Learners conduct a mock internal audit of a laboratory section using a checklist and write an audit report. (4h)
  • Root Cause Analysis and CAPA — Learners analyse a given non-conformance scenario, perform root cause analysis, and develop a CAPA plan. (4h)

Summatives

Each delegate is assessed continuously throughout the course via daily exercises, scored practical assignments, and a final summative test at the end.

Practical Assignments — 30%

Practical assignments are observed and scored against a rubric during the practical sessions. Each delegate's practical mark is averaged into a single 100% score and contributes 30% to the final total.

Daily Exercises — 20%

Every training day ends with a multiple-choice exercise scored out of 100%. The scores from each daily exercise are averaged across the duration of the course to produce a Daily Average mark, which contributes 20% to the final total.

Final Test — 50%

On the last day a final summative test is written. It is a multiple-choice paper with multiple-answer questions: each question may have more than one correct option, and a single wrong selection on a question marks the entire question wrong — no partial credit. The final test is scored out of 100% and contributes 50% to the overall mark.

Certificate of Completion

Awarded to delegates who achieve an overall mark of 50% or higher on the Final Total (Practicals 30% + Daily Average 20% + Final Test 50%).
