This course equips learners with the knowledge and skills to systematically identify, analyze, and prevent equipment failures in industrial settings. Participants will learn to apply root cause analysis techniques and implement preventive strategies to enhance equipment reliability and reduce downtime. The course aims to foster a proactive maintenance culture aligned with South African industry standards.

Course Outline

Day 1: Fundamentals of Equipment Failure

Objectives:
• Define equipment failure and its economic impact
• Identify common failure modes in industrial equipment
• Explain the role of maintenance in failure prevention
• Distinguish between random and wear-out failures
• Introduce basic failure statistics and reliability concepts

Topics:
• Introduction to equipment failure: definitions and costs
• Failure modes and mechanisms (wear, corrosion, fatigue, fracture)
• Basic reliability concepts: MTBF, MTTF, failure rate
• The bathtub curve and failure patterns
• Overview of maintenance strategies (corrective, preventive, predictive)
• Case study: common failures in rotating equipment
• Introduction to failure reporting and documentation
• Q&A and group discussion

Day 2: Failure Analysis Techniques

Objectives:
• Apply systematic failure investigation methods
• Use root cause analysis (RCA) tools effectively
• Interpret visual and microscopic evidence of failure
• Identify material and design-related causes
• Document findings in a structured failure report

• Failure analysis process: data collection, examination, testing
• Visual inspection and fractography basics
• Non-destructive testing (NDT) methods for failure analysis
• Root cause analysis tools: 5 Whys, fishbone diagram, fault tree
• Material analysis: metallography, hardness testing, SEM
• Case study: bearing failure analysis
• Writing a failure analysis report
• Group exercise: RCA on a given failure scenario

Day 3: Prevention Strategies – Design and Operation

Objectives:
• Apply design principles to reduce failure risk
• Implement preventive maintenance schedules
• Use condition monitoring techniques for early detection
• Understand lubrication and contamination control
• Develop standard operating procedures (SOPs) for critical equipment

• Design for reliability: FMEA and design reviews
• Preventive maintenance: planning and scheduling
• Condition monitoring: vibration analysis, thermography, oil analysis
• Lubrication best practices and contamination control
• Standard operating procedures and operator training
• Human factors in equipment operation

Day 4: Advanced Prevention – Reliability Engineering

Objectives:
• Apply Reliability Centered Maintenance (RCM) methodology
• Use Weibull analysis for life data interpretation
• Implement predictive maintenance technologies
• Calculate and improve equipment availability
• Develop a continuous improvement plan for failure reduction

• Reliability Centered Maintenance (RCM) framework
• Weibull analysis: parameter estimation and interpretation
• Predictive maintenance: infrared, ultrasonic, motor current analysis
• Availability, OEE, and reliability metrics
• Life cycle cost analysis
• Root cause failure analysis (RCFA) follow-up
• Case study: implementing RCM on a conveyor system
• Workshop: Weibull analysis using sample data

Day 5: Integration and Continuous Improvement

Objectives:
• Develop a failure prevention program for an organization
• Integrate analysis findings into maintenance strategies
• Use data-driven decision making for reliability improvement
• Foster a culture of failure prevention and continuous improvement
• Present a capstone project on failure analysis and prevention

• Building a failure prevention program: policy and procedures
• Data management and reliability databases
• Key performance indicators for failure prevention

Practicals

16 hours of practicals To be conducted online or on-campus or in-house
Overview

Hands-on practicals reinforce failure analysis techniques and prevention strategies. Learners will perform visual inspection, collect and interpret condition monitoring data, apply root cause analysis tools, and develop preventive maintenance plans using real equipment examples.

Practical Activities
  • Visual Inspection and Fracture Analysis — Learners examine failed components (shafts, bearings, gears) using visual and low-magnification inspection to identify fracture modes and surface features. (4h)
  • Non-Destructive Testing (NDT) Demonstration — Learners perform dye penetrant and magnetic particle inspection on cracked samples to detect surface and near-surface defects. (3h)
  • Root Cause Analysis (RCA) Workshop — In teams, learners conduct a root cause analysis on a simulated pump failure using the 5 Whys and fishbone diagram, then present findings. (4h)
  • Condition Monitoring Data Interpretation — Learners analyze vibration spectra and oil analysis reports from rotating equipment to identify faults and recommend maintenance actions. (3h)
  • Preventive Maintenance Plan Development — Using a case study, learners develop a preventive maintenance plan for a critical piece of equipment, including task lists and frequencies. (2h)

Summatives

Each delegate is assessed continuously throughout the course via daily exercises, scored practical assignments, and a final summative test at the end.

Practical Assignments — 30%

Practical assignments are observed and scored against a rubric during the practical sessions. Each delegate's practical mark is averaged into a single 100% score and contributes 30% to the final total.

Daily Exercises — 20%

Every training day ends with a multiple-choice exercise scored out of 100%. The scores from each daily exercise are averaged across the duration of the course to produce a Daily Average mark, which contributes 20% to the final total.

Final Test — 50%

On the last day a final summative test is written. It is a multiple-choice paper with multiple-answer questions: each question may have more than one correct option, and a single wrong selection on a question marks the entire question wrong — no partial credit. The final test is scored out of 100% and contributes 50% to the overall mark.

Certificate

Certificate of Completion

Awarded to delegates who achieve an overall mark of 50% or higher on the Final Total (Practicals 30% + Daily Average 20% + Final Test 50%).
