Description

This course equips team leaders and members with the skills to plan, conduct, and participate in Hazard and Operability (HAZOP) studies. Participants learn to systematically identify process hazards, assess risks, and recommend controls, ensuring compliance with South African health and safety regulations.

Target Audience

Team leaders, supervisors, engineers, and safety personnel who are or will be members of HAZOP study teams in process industries such as petrochemical, chemical, pharmaceutical, and mining sectors.

Course Outline

Day 1: Introduction to HAZOP and Process Safety

Objectives:
• Understand the purpose and principles of HAZOP studies
• Identify key process safety concepts and terminology
• Recognize the role of HAZOP in risk management
• Describe the HAZOP team composition and responsibilities

Topics:
• Overview of process safety and major accident hazards
• History and evolution of HAZOP methodology
• Key HAZOP terminology (nodes, deviations, parameters, guide words)
• HAZOP study phases: preparation, examination, documentation, follow-up
• Team roles: leader, scribe, team members (process, operations, maintenance, etc.)
• Introduction to risk matrix and risk ranking

Day 2: HAZOP Methodology and Preparation

Objectives:
• Apply systematic HAZOP methodology to identify hazards
• Prepare for a HAZOP study: select nodes, define design intent, gather data
• Use guide words and parameters to generate deviations
• Document deviations, causes, consequences, and safeguards

Topics:
• Step-by-step HAZOP procedure: select node, describe design intent, identify deviations
• Guide words (No, More, Less, Reverse, etc.) and process parameters (flow, pressure, temperature, etc.)
• Generating meaningful deviations and brainstorming causes
• Identifying consequences and existing safeguards
• Introduction to HAZOP worksheets
• Pre-study preparation: piping and instrumentation diagrams (P&IDs), process flow diagrams (PFDs), operating procedures

Day 3: Advanced HAZOP Techniques and Team Leadership

Objectives:
• Lead a HAZOP session effectively, managing team dynamics
• Apply advanced techniques for complex scenarios
• Evaluate safeguard adequacy and recommend actions
• Document study outcomes clearly and concisely

Topics:
• Leadership skills for HAZOP facilitators: keeping focus, encouraging participation, handling conflict
• Advanced deviations: combination deviations, time-dependent deviations, human factors
• Evaluating safeguard effectiveness: independence, reliability, auditability
• Developing recommendations: hierarchy of controls, action items
• Managing documentation: recording decisions, assumptions, and action tracking
• Dealing with common challenges: incomplete data, scope creep, team fatigue

Day 4: Practical HAZOP Workshop – Case Study 1

Objectives:
• Conduct a complete HAZOP study on a simple batch process
• Apply all phases: preparation, examination, documentation, and follow-up
• Generate a risk-ranked action plan
• Demonstrate effective team communication and leadership

Topics:
• Review of case study process: batch reactor system with P&IDs and operating procedures
• Node selection and design intent definition
• Systematic deviation analysis using guide words and parameters
• Risk ranking using company risk matrix
• Formulating recommendations and assigning responsibilities
• Preparation of final HAZOP report

Day 5: Practical HAZOP Workshop – Case Study 2 and Integration

Objectives:
• Lead a HAZOP study on a continuous process with multiple nodes
• Integrate HAZOP findings into a process safety management system
• Apply lessons learned to improve future HAZOP studies
• Review and critique HAZOP reports for quality and completeness

Topics:
• Case study 2: continuous distillation column with reboiler and condenser
• Multi-node HAZOP with interlinked systems
• Linking HAZOP outcomes to layers of protection analysis (LOPA)
• Integration with safety instrumented systems (SIS) and alarm management
• Quality assurance of HAZOP studies: peer review and audit
• Course wrap-up and assessment

Overview

Practical workshops form the core of this course, allowing learners to apply HAZOP methodology to realistic process scenarios. Learners work in teams to conduct full HAZOP studies on batch and continuous processes, from preparation through to documentation and action planning, under the guidance of an experienced facilitator.

Practical Activities
  • HAZOP Study on Batch Reactor System — Learners, in teams, conduct a HAZOP study on a batch reactor process using provided P&IDs and operating procedures. They identify nodes, deviations, causes, consequences, safeguards, and recommend actions, completing a HAZOP worksheet and risk ranking. (8h)
  • HAZOP Study on Continuous Distillation Column — Learners lead and participate in a HAZOP study on a continuous distillation column, handling multiple nodes and interconnections. They produce a final report with risk-ranked actions and present findings for peer review. (8h)

Summatives

Each delegate is assessed continuously throughout the course via daily exercises, scored practical assignments, and a final summative test at the end.

Practical Assignments — 30%

Practical assignments are observed and scored against a rubric during the practical sessions. Each delegate's practical mark is averaged into a single 100% score and contributes 30% to the final total.

Daily Exercises — 20%

Every training day ends with a multiple-choice exercise scored out of 100%. The scores from each daily exercise are averaged across the duration of the course to produce a Daily Average mark, which contributes 20% to the final total.

Final Test — 50%

On the last day a final summative test is written. It is a multiple-choice paper with multiple-answer questions: each question may have more than one correct option, and a single wrong selection on a question marks the entire question wrong — no partial credit. The final test is scored out of 100% and contributes 50% to the overall mark.

Certificate

Certificate of Completion

Awarded to delegates who achieve an overall mark of 50% or higher on the Final Total (Practicals 30% + Daily Average 20% + Final Test 50%).
