Description

This course provides a foundational understanding of Hazard and Operability (HAZOP) studies, a systematic technique for identifying potential hazards and operability problems in process industries. Participants will learn the principles, methodology, and application of HAZOP as a risk assessment tool, enabling them to contribute effectively to HAZOP teams and improve process safety.

Learning Outcomes

  • Apply the HAZOP methodology, including guide words, deviations, and node selection, to systematically identify hazards.
  • Analyze process parameters and deviations to evaluate potential causes and consequences of hazards.

  • Demonstrate effective participation in a HAZOP team, including communication and decision-making skills.
  • Evaluate the role of HAZOP within a broader risk management framework, including ALARP principles.

Target Audience

Process engineers, safety practitioners, operations supervisors, and technical managers involved in process design, operation, or maintenance in industries such as chemical, petrochemical, pharmaceutical, and mining.

Prerequisites

None — open enrollment. However, familiarity with process operations or engineering is beneficial.

Course Outline

Day 1: Introduction to Hazard and Operability Studies

Objectives:
• Understand the origins and purpose of HAZOP studies
• Identify key roles and responsibilities in a HAZOP team
• Recognise when a HAZOP study is required
• Explain basic HAZOP terminology (nodes, deviations, causes, consequences, safeguards)
• Differentiate HAZOP from other risk assessment techniques

Topics:
• History and development of HAZOP methodology
• Regulatory and industry drivers (SAQA, OHSA, SANS)
• HAZOP team composition and leadership
• Overview of the HAZOP process: preparation, review, reporting
• Introduction to guide words and process parameters
• Difference between HAZOP, FMEA, and What-If analysis

Day 2: HAZOP Methodology and Preparation

Objectives:
• Prepare a HAZOP study: scope, documentation, and scheduling
• Define nodes and design intent
• Apply guide words (NONE, MORE, LESS, REVERSE, etc.) to parameters
• Identify credible causes and consequences
• Document findings using standard HAZOP worksheets

Topics:
• Step-by-step HAZOP procedure
• Selecting and defining nodes (P&ID, PFD review)
• Guide word and parameter combinations
• Brainstorming deviations systematically
• Using the HAZOP worksheet: columns and entries
• Pre-HAZOP data gathering: P&IDs, operating procedures, equipment specs

Day 3: Conducting the HAZOP Review Meeting

Objectives:
• Facilitate a HAZOP review session effectively
• Elicit input from all team members
• Evaluate existing safeguards and risk levels
• Propose recommendations for risk reduction
• Manage time and scope creep during the meeting

Topics:
• Chairperson and scribe roles in action
• Techniques for encouraging participation
• Risk ranking: likelihood, severity, and risk matrix
• Defining safeguards: engineering, administrative, PPE
• Formulating clear, actionable recommendations
• Handling common challenges: scope creep, fatigue, conflicts

Day 4: Advanced HAZOP Techniques and Specific Applications

Objectives:
• Apply HAZOP to batch processes and non-process systems
• Understand HAZOP for computer-controlled systems (CHAZOP)
• Integrate layers of protection analysis (LOPA) with HAZOP
• Adapt HAZOP for procedural and human factors
• Review case studies from South African industries

Topics:
• HAZOP for batch vs. continuous processes
• CHAZOP: guide words for software and hardware
• Combining HAZOP and LOPA for SIL determination
• Human factors in HAZOP: task analysis and error modes
• HAZOP of procedures and emergency shutdown systems
• Industry case studies: petrochemical, mining, pharmaceuticals

Day 5: HAZOP Reporting, Follow-up, and Integration

Objectives:
• Compile a comprehensive HAZOP report
• Develop action plans and track recommendations
• Close out HAZOP actions effectively
• Integrate HAZOP into the safety lifecycle
• Prepare for a HAZOP revalidation or study update

Topics:
• Report structure: executive summary, worksheets, action register
• Assigning responsibilities and target dates
• Management of change (MOC) and HAZOP updates
• Revalidation studies: when and how to re-HAZOP
• Auditing the HAZOP process
• Course wrap-up and assessment

Practicals

16 hours of practicals To be conducted online or on-campus or in-house
Overview

Hands-on practical sessions allow learners to apply HAZOP methodology to real-world process scenarios. Learners will work in teams to prepare, facilitate, and document a mini-HAZOP study, reinforcing the theoretical concepts covered in class.

Practical Activities
  • Practical 1: Node Identification and Deviation Brainstorming — Given a P&ID of a simple process (e.g., cooling water system), learners identify nodes, select guide word-parameter pairs, and brainstorm possible deviations. (4h)
  • Practical 2: Facilitated HAZOP Review Session — Learners role-play as chairperson, scribe, and team members to conduct a HAZOP review on a selected node, identifying causes, consequences, safeguards, and recommendations. (6h)
  • Practical 3: HAZOP Report Compilation and Action Tracking — Using the outputs from Practical 2, learners compile a HAZOP report including worksheets, action register, and executive summary, then simulate follow-up tracking. (4h)
  • Practical 4: Integrated HAZOP Revalidation Exercise — Learners review an existing HAZOP study for a modified process and decide on revalidation scope, updating the study as needed. (2h)

Summatives

Each delegate is assessed continuously throughout the course via daily exercises, scored practical assignments, and a final summative test at the end.

Practical Assignments — 30%

Practical assignments are observed and scored against a rubric during the practical sessions. Each delegate's practical mark is averaged into a single 100% score and contributes 30% to the final total.

Daily Exercises — 20%

Every training day ends with a multiple-choice exercise scored out of 100%. The scores from each daily exercise are averaged across the duration of the course to produce a Daily Average mark, which contributes 20% to the final total.

Final Test — 50%

On the last day a final summative test is written. It is a multiple-choice paper with multiple-answer questions: each question may have more than one correct option, and a single wrong selection on a question marks the entire question wrong — no partial credit. The final test is scored out of 100% and contributes 50% to the overall mark.

Certificate

Certificate of Completion

Awarded to delegates who achieve an overall mark of 50% or higher on the Final Total (Practicals 30% + Daily Average 20% + Final Test 50%).
