Course Outline

Day 1: Foundations of Incident Investigation

Objectives:
• Understand the purpose and principles of incident investigation.
• Define key terms: incident, accident, near miss, hazard, risk.
• Explain the legal and regulatory framework in South Africa.
• Identify the roles and responsibilities of investigators.
• Recognise the benefits of a systematic investigation process.

• Legal framework: OHS Act, regulations, and standards.
• The incident investigation process: overview.
• Types of incidents: accidents, incidents, near misses.
• Human factors and error types.
• The role of the investigator: skills and ethics.

Day 2: Planning and Scene Management

Objectives:
• Develop an investigation plan.
• Secure and manage the incident scene.
• Collect and preserve physical evidence.
• Identify and interview witnesses.
• Document the scene through notes, photos, and sketches.

• Pre-investigation planning: scope, resources, team.
• Scene security and safety: first actions.
• Evidence identification and preservation.
• Photography and sketching techniques.
• Witness identification and interviewing basics.
• Documenting observations: logbooks and forms.
• Practical exercise: mock scene assessment.
• Group activity: developing an investigation plan.

Day 3: Data Collection and Analysis Techniques

Objectives:
• Apply systematic data collection methods.
• Use root cause analysis tools (e.g., 5 Whys, Fishbone).
• Analyse evidence to identify direct, contributing, and root causes.
• Evaluate human factors and system failures.
• Distinguish between symptoms and root causes.

• Data collection: documents, records, interviews.
• Interview techniques: open-ended questions, active listening.
• Root cause analysis: 5 Whys, Fishbone diagram.
• Change analysis and barrier analysis.
• Human factors analysis: error types, performance.
• Systematic cause analysis techniques.
• Practical exercise: applying root cause analysis to a case.
• Group discussion: common analysis pitfalls.

Day 4: Reporting and Corrective Actions

Objectives:
• Structure a professional investigation report.
• Develop effective corrective and preventive actions.
• Prioritise recommendations based on risk.
• Communicate findings to stakeholders.
• Understand the hierarchy of controls.

Topics:
• Report writing: structure, clarity, objectivity.
• Developing findings and conclusions.
• Corrective action planning: SMART criteria.
• Hierarchy of controls: elimination to PPE.
• Presenting recommendations to management.
• Legal considerations in reporting.
• Practical exercise: drafting a report from a case study.
• Role-play: presenting findings to a safety committee.

Day 5: Implementation, Review, and Continuous Improvement

Objectives:
• Implement corrective actions and track effectiveness.
• Conduct follow-up investigations and trend analysis.
• Integrate investigation findings into safety management systems.
• Review and improve the investigation process.
• Prepare for external audits and inspections.

• Action tracking and verification.

Practicals

Overview

The practicals provide hands-on experience in scene management, evidence collection, root cause analysis, and report writing. Learners will apply investigation techniques to realistic scenarios, reinforcing theoretical knowledge and developing practical skills.

Practical Activities
  • Mock Incident Scene Investigation — Learners work in teams to secure a simulated incident scene, collect physical evidence, take photographs, and create sketches. (4h)
  • Witness Interviewing Exercise — Learners conduct mock witness interviews using open-ended questioning and active listening to gather factual information. (3h)
  • Root Cause Analysis Workshop — Using a detailed case study, learners apply 5 Whys and Fishbone diagram techniques to identify root causes and contributing factors. (4h)
  • Investigation Report Drafting — Learners draft a complete investigation report from a provided scenario, including findings, conclusions, and recommendations. (3h)
  • Full Investigation Simulation — Teams conduct a complete simulated investigation from scene to report, then present findings to the class for feedback. (2h)

Summatives

Each delegate is assessed continuously throughout the course via daily exercises, scored practical assignments, and a final summative test at the end.

Practical Assignments — 30%

Practical assignments are observed and scored against a rubric during the practical sessions. Each delegate's practical mark is averaged into a single 100% score and contributes 30% to the final total.

Daily Exercises — 20%

Every training day ends with a multiple-choice exercise scored out of 100%. The scores from each daily exercise are averaged across the duration of the course to produce a Daily Average mark, which contributes 20% to the final total.

Final Test — 50%

On the last day a final summative test is written. It is a multiple-choice paper with multiple-answer questions: each question may have more than one correct option, and a single wrong selection on a question marks the entire question wrong — no partial credit. The final test is scored out of 100% and contributes 50% to the overall mark.

Certificate of Completion

Awarded to delegates who achieve an overall mark of 50% or higher on the Final Total (Practicals 30% + Daily Average 20% + Final Test 50%).
