Course Outline

Day 1: Foundations of the Therapeutic Environment

Objectives:
• Define the concept of a therapeutic environment in mental health care.
• Identify the core components that contribute to a healing milieu.
• Explain the impact of physical, social, and psychological factors on patient well-being.
• Recognise the role of the multidisciplinary team in creating a therapeutic setting.

• Introduction to therapeutic environments: historical context and current perspectives.
• Key elements: safety, structure, support, and empowerment.
• The physical environment: design, lighting, noise, and space.
• Social environment: staff attitudes, communication, and peer interactions.
• Psychological environment: predictability, autonomy, and respect.
• Overview of the multidisciplinary team and their contributions.

Day 2: Safety and Crisis Prevention

Objectives:
• Identify potential safety risks in mental health settings.
• Apply de-escalation techniques to prevent crises.
• Implement environmental modifications to enhance safety.
• Understand legal and ethical considerations in managing aggression.

Topics:
• Risk assessment: environmental, patient, and staff factors.
• De-escalation strategies: verbal and non-verbal communication.
• Environmental safety: furniture, access control, and observation levels.
• Managing aggression and violence: policies and procedures.
• Legal frameworks: Mental Health Care Act and patient rights.
• Ethical dilemmas in restrictive interventions.

Day 3: Patient-Centred Care and Therapeutic Communication

Objectives:
• Apply principles of patient-centred care in daily interactions.
• Use therapeutic communication techniques to build rapport.
• Facilitate patient involvement in treatment planning.
• Adapt communication for diverse patient populations.

Topics:
• Principles of patient-centred care: respect, dignity, and collaboration.
• Therapeutic communication: active listening, empathy, and validation.
• Motivational interviewing techniques.
• Cultural competence in mental health settings.
• Involving patients in goal setting and decision making.
• Documentation and confidentiality considerations.

Day 4: Activity and Engagement in the Therapeutic Environment

Objectives:
• Design structured activities that promote engagement and recovery.
• Implement routines that balance rest, activity, and therapy.
• Evaluate the role of occupational and recreational therapy.
• Adapt activities for patients with varying levels of functioning.

Topics:
• The importance of structure and routine in mental health.
• Types of therapeutic activities: groups, individual tasks, and recreation.
• Occupational therapy principles and applications.
• Recreational therapy: benefits and implementation.
• Activity scheduling and time management on the unit.
• Evaluating patient responses and adjusting interventions.

Day 5: Integration, Evaluation, and Quality Improvement

Objectives:
• Integrate all components to create a cohesive therapeutic environment.
• Evaluate the effectiveness of the therapeutic milieu.
• Identify areas for quality improvement and implement changes.
• Develop a personal action plan for sustaining a therapeutic environment.

Topics:
• Case studies: applying the therapeutic environment model.
• Assessment tools for evaluating the milieu.
• Quality improvement cycles: plan-do-study-act.
• Staff self-care and preventing burnout.
• Creating a culture of continuous improvement.
• Action planning for implementation in own practice.

Practicals

This course is made up of exercises and case studies delivered alongside the theory — no separate practical sessions are required to complete it.

Summatives

Each delegate is assessed continuously throughout the course and a final summative test at the end.

Daily Exercises — 30%

Every training day ends with a multiple-choice exercise scored out of 100%. The scores from each daily exercise are averaged across the duration of the course to produce a Daily Average mark, which contributes 30% to the final total.

Final Test — 70%

On the last day a final summative test is written. It is a multiple-choice paper with multiple-answer questions: each question may have more than one correct option, and a single wrong selection on a question marks the entire question wrong — no partial credit. The final test is scored out of 100% and contributes 70% to the overall mark.

All marks are recorded on the AATICD LMS and visible to each learner under their account.

Certificate

Certificate of Completion

Awarded to delegates who achieve an overall mark of 50% or higher on the Final Total (Daily Average 30% + Final Test 70%).
