This course equips healthcare professionals and laboratory personnel with foundational knowledge of microorganism structure and biology, enabling them to apply this understanding in clinical practice. Target Audience

This course is designed for clinical laboratory technicians, nurses, medical students, and other healthcare practitioners who require a working knowledge of microbiology in their daily practice.

Course Outline

Day 1: Introduction to Microbiology and Microbial Classification

Objectives:
• Define microbiology and its relevance to clinical practice
• Describe the major groups of microorganisms
• Explain the principles of microbial classification and nomenclature
• Identify the key differences between prokaryotes and eukaryotes

Topics:
• Introduction to microbiology: history and scope
• Overview of microorganisms: bacteria, viruses, fungi, parasites
• Prokaryotic vs. eukaryotic cell structure
• Classification systems: Bergey's Manual, Whittaker's five kingdoms
• Nomenclature and taxonomy basics

Day 2: Bacterial Cell Structure and Function

Objectives:
• Describe the structure and function of bacterial cell wall, cell membrane, and cytoplasmic components
• Differentiate between Gram-positive and Gram-negative cell walls
• Explain the role of bacterial surface structures (capsule, flagella, pili)
• Discuss bacterial spore formation and its clinical significance

Topics:
• Bacterial cell wall: peptidoglycan, Gram staining mechanism
• Cell membrane: structure, transport, and energy generation
• Cytoplasmic components: ribosomes, nucleoid, plasmids, inclusions
• Surface structures: capsule, flagella, pili, fimbriae
• Endospores: structure, formation, and resistance

Day 3: Bacterial Growth, Metabolism, and Genetics

Objectives:
• Outline bacterial growth phases and factors affecting growth
• Describe basic bacterial metabolism: aerobic, anaerobic, fermentation
• Explain bacterial DNA replication, transcription, and translation
• Discuss genetic exchange mechanisms: transformation, transduction, conjugation

Topics:
• Bacterial growth curve: lag, log, stationary, death phases
• Factors influencing growth: temperature, pH, oxygen, nutrients
• Overview of bacterial metabolism: catabolism, anabolism
• Bacterial genetics: chromosome, plasmids, transposons
• Horizontal gene transfer: transformation, transduction, conjugation

Day 4: Viruses: Structure, Replication, and Pathogenesis

Objectives:
• Describe the general structure of viruses and classification
• Explain the lytic and lysogenic replication cycles
• Discuss viral pathogenesis and host-virus interactions
• Identify common clinically important viruses

Topics:
• Virus structure: capsid, envelope, nucleic acid types
• Classification: Baltimore system, ICTV taxonomy
• Replication cycles: lytic, lysogenic, and persistent infections
• Viral pathogenesis: entry, tropism, immune evasion
• Examples: HIV, influenza, hepatitis viruses, herpesviruses

Day 5: Fungi and Parasites: Structure and Biology

Objectives:
• Describe the structural features of fungi: yeasts, molds, dimorphic
• Explain fungal reproduction and pathogenesis
• Differentiate between protozoa, helminths, and ectoparasites
• Discuss the life cycles of clinically important parasites

Topics:
• Fungal cell structure: cell wall, hyphae, spores
• Fungal reproduction: asexual and sexual
• Medically important fungi: Candida, Aspergillus, dermatophytes
• Parasite classification: protozoa, helminths, arthropods
• Life cycles: Plasmodium, Entamoeba, Schistosoma, hookworms

Day 6: Host-Microbe Interactions and Immunology Basics

Objectives:
• Differentiate between commensalism, mutualism, and parasitism
• Describe the normal flora and its role in health
• Explain the innate immune response to microbes
• Outline adaptive immunity: humoral and cell-mediated

Topics:
• Host-microbe relationships: symbiosis, pathogenicity, virulence
• Normal microbiota: skin, gut, respiratory, urogenital
• Innate immunity: barriers, phagocytes, complement, inflammation
• Adaptive immunity: antigens, antibodies, T cells, B cells
• Immune evasion strategies by pathogens

Day 7: Microbial Pathogenesis and Mechanisms of Disease

Objectives:
• Explain how microorganisms cause disease: Koch's postulates, molecular Koch's postulates
• Describe virulence factors: adhesins, toxins, enzymes
• Discuss biofilm formation and its clinical impact
• Outline the stages of infectious disease process

Topics:
• Pathogenicity vs. virulence: definitions and concepts
• Adhesion and colonization: fimbriae, biofilms
• Toxins: exotoxins, endotoxins, mode of action
• Enzymes and invasins: hyaluronidase, coagulase, collagenase
• Stages of infection: incubation, prodromal, acute, convalescence

Day 8: Antimicrobial Agents and Resistance

Objectives:
• Classify antimicrobial agents by mechanism of action
• Describe mechanisms of antimicrobial resistance
• Explain the principles of antimicrobial susceptibility testing
• Discuss strategies to combat antimicrobial resistance

Topics:
• Antibiotic classes: cell wall, protein synthesis, nucleic acid, metabolic inhibitors
• Mechanisms of resistance: efflux pumps, enzyme inactivation, target modification
• Susceptibility testing: disk diffusion, MIC, Etest
• Multidrug-resistant organisms: MRSA, VRE, ESBL, CRE
• Antimicrobial stewardship: principles and implementation

Day 9: Clinical Applications and Laboratory Integration

Objectives:
• Apply knowledge of microbial structure and biology to specimen collection and handling
• Interpret basic laboratory results for common infections
• Discuss infection prevention and control measures
• Integrate microbiology principles into clinical decision-making

Topics:
• Specimen collection and transport: blood, urine, sputum, swabs
• Overview of diagnostic methods: microscopy, culture, serology, molecular
• Case studies: respiratory, urinary, wound, bloodstream infections
• Infection control: hand hygiene, isolation precautions, sterilization
• Role of microbiology in antibiotic stewardship and patient management

Practicals

Overview

Practical sessions provide hands-on experience in aseptic technique, Gram staining, culture methods, antibiotic susceptibility testing, and specimen handling. Learners will apply theoretical knowledge to simulate clinical laboratory procedures, reinforcing skills essential for infection control and diagnosis.

Practical Activities
  • Aseptic Technique and Culture Methods — Learners practice aseptic transfer of microorganisms, streak plating, and inoculation of liquid and solid media using bacterial cultures. (8h)
  • Gram Staining and Microscopy — Learners perform Gram staining on known and unknown bacterial samples and observe morphological characteristics under the microscope. (6h)
  • Antibiotic Susceptibility Testing (Kirby-Bauer) — Learners perform disk diffusion susceptibility testing on bacterial cultures, measure zones of inhibition, and interpret results using CLSI standards. (6h)
  • Specimen Collection and Handling Simulation — Using simulated specimens, learners practice proper collection, labeling, transport, and processing of samples for microbiological analysis. (4h)
  • Case-Based Laboratory Interpretation — Learners review case studies with laboratory results (culture, Gram stain, susceptibility) and discuss appropriate clinical responses and infection control measures. (6h)

Summatives

Each delegate is assessed continuously throughout the course via daily exercises, scored practical assignments, and a final summative test at the end.

Practical Assignments — 30%

Practical assignments are observed and scored against a rubric during the practical sessions. Each delegate's practical mark is averaged into a single 100% score and contributes 30% to the final total.

Daily Exercises — 20%

Every training day ends with a multiple-choice exercise scored out of 100%. The scores from each daily exercise are averaged across the duration of the course to produce a Daily Average mark, which contributes 20% to the final total.

Final Test — 50%

On the last day a final summative test is written. It is a multiple-choice paper with multiple-answer questions: each question may have more than one correct option, and a single wrong selection on a question marks the entire question wrong — no partial credit. The final test is scored out of 100% and contributes 50% to the overall mark.

Certificate of Completion

Awarded to delegates who achieve an overall mark of 50% or higher on the Final Total (Practicals 30% + Daily Average 20% + Final Test 50%).
