This course equips healthcare providers with the essential knowledge and practical skills to deliver comprehensive postpartum care to both mother and neonate. It focuses on promoting maternal recovery, neonatal well-being, and early detection of complications within the first six weeks after birth.

Learning Outcomes

  • Demonstrate the ability to perform a systematic postpartum assessment of the mother, including vital signs, uterine involution, lochia, and perineal healing.
  • Apply evidence-based guidelines to monitor neonatal vital signs, feeding, elimination, and umbilical cord care.
  • Evaluate maternal and neonatal risk factors for common postpartum complications such as haemorrhage, infection, and jaundice.
  • Implement culturally sensitive health education on breastfeeding, contraception, and postnatal warning signs.
  • Design a personalised postpartum care plan that addresses the physical, emotional, and social needs of the mother-neonate dyad.
  • Analyze the legal and ethical responsibilities of the healthcare provider in documenting and reporting postpartum findings.

Target Audience

Midwives, enrolled nurses, nursing assistants, and community health workers who provide direct postpartum care in hospital or primary healthcare settings.

Prerequisites

None — open enrollment. However, learners must be currently employed or volunteering in a healthcare setting that provides maternal and neonatal care.

Course Outline

Day 1: Introduction to Postpartum Care

Objectives:
• Understand the physiological changes in the mother after childbirth
• Identify the key components of immediate postpartum care
• Recognize the importance of family-centered care
• Describe the role of the healthcare provider in postpartum support

Topics:
• Overview of postpartum period (first 6 weeks)
• Maternal physiological adaptations (uterine involution, lochia, vital signs)
• Neonatal transition to extrauterine life
• Principles of family-centered care
• Communication and cultural sensitivity

Day 2: Maternal Physical Assessment and Monitoring

Objectives:
• Perform a systematic postpartum maternal assessment
• Monitor vital signs and interpret deviations
• Assess uterine involution, lochia, and perineal healing
• Identify signs of complications (e.g., hemorrhage, infection)

Topics:
• Postpartum vital signs and pain assessment
• Uterine fundal height and consistency
• Lochia assessment (color, amount, odor)
• Perineal care and episiotomy assessment
• Breast examination and engorgement
• Screening for postpartum complications

Day 3: Neonatal Assessment and Care

Objectives:
• Conduct a comprehensive newborn physical assessment
• Perform routine neonatal care (cord care, eye care, vitamin K)
• Recognize normal newborn behaviors and reflexes
• Identify early signs of neonatal illness

Topics:
• Newborn vital signs and weight
• Head-to-toe assessment (fontanelles, skin, heart, lungs, abdomen)
• Umbilical cord care and hygiene
• Newborn reflexes and behavior
• Screening for jaundice, hypoglycemia, and infection
• Immunization and prophylactic treatments

Day 4: Breastfeeding and Infant Feeding Support

Objectives:
• Promote and support exclusive breastfeeding
• Assist with correct latch and positioning
• Identify and manage common breastfeeding challenges
• Understand alternative feeding methods when needed

Topics:
• Benefits of breastfeeding and WHO recommendations
• Anatomy of lactation and milk production
• Latch assessment and positioning techniques
• Common issues: sore nipples, engorgement, mastitis
• Hand expression and breast pump use
• Formula feeding preparation and safety

Day 5: Postpartum Mental Health and Psychosocial Support

Objectives:
• Identify risk factors and symptoms of postpartum depression
• Provide emotional support and screening
• Understand the impact of social determinants on postpartum health
• Refer to appropriate mental health resources

Topics:
• Baby blues vs. postpartum depression vs. psychosis
• Edinburgh Postnatal Depression Scale (EPDS)
• Partner and family support
• Coping strategies and self-care
• Social support systems and community resources
• Impact of trauma, abuse, and substance use

Day 6: Nutrition, Hygiene, and Lifestyle in the Postpartum Period

Objectives:
• Advise on maternal nutritional needs during breastfeeding
• Promote perineal and wound hygiene
• Discuss safe physical activity and rest
• Educate on family planning and contraception options

Topics:
• Dietary requirements for lactating mothers
• Hydration and supplements (iron, calcium)
• Perineal hygiene and wound care
• Postpartum exercise and pelvic floor health
• Sleep deprivation management
• Contraception counseling and birth spacing

Day 7: Complications and Emergency Management

Objectives:
• Recognize signs of postpartum hemorrhage, preeclampsia, and sepsis
• Initiate emergency management protocols
• Understand neonatal resuscitation basics
• Coordinate referral and transport

Topics:
• Postpartum hemorrhage: causes, assessment, and management
• Hypertensive disorders: preeclampsia and eclampsia
• Postpartum infections: endometritis, mastitis, wound infection
• Neonatal emergencies: respiratory distress, hypothermia, hypoglycemia
• Basic life support for mother and neonate
• Emergency drills and team communication

Day 8: Community-Based Postpartum Care and Health Promotion

Objectives:
• Plan home visits and follow-up schedules
• Educate families on danger signs and when to seek care
• Promote immunization and well-baby checks
• Link families with community health services

• Postpartum home visit protocols
• Maternal and child health record keeping
• Health education on danger signs (mother and baby)
• Immunization schedule and well-baby visits
• Support groups and community health workers
• Referral pathways and continuity of care

Day 9: Integration, Case Studies, and Assessment

Objectives:
• Apply knowledge to complex case scenarios
• Demonstrate integrated postpartum care skills
• Evaluate learning through formative and summative assessments
• Develop a personal action plan for practice improvement

• Case studies: maternal hemorrhage, neonatal jaundice, postpartum depression

Practicals

28 hours of practicals To be conducted online or on-campus or in-house
Overview

Learners will practice clinical skills in a simulated or clinical setting, including maternal and neonatal assessments, breastfeeding support, and emergency drills. Practicals reinforce theoretical knowledge and ensure competency in essential postpartum care procedures.

Practical Activities
  • Maternal Physical Assessment — Learners perform systematic postpartum maternal assessments on mannequins or standardized patients, including vital signs, uterine palpation, and perineal inspection. (6h)
  • Newborn Assessment and Care — Learners conduct a head-to-toe newborn assessment, perform cord care, administer vitamin K and eye prophylaxis, and practice safe handling and swaddling. (6h)
  • Breastfeeding Support and Latch Assessment — Learners assist mothers with breastfeeding positioning and latch assessment using models or simulated patients, and provide guidance on common challenges. (6h)
  • Postpartum Emergency Drills — Learners participate in simulated emergency scenarios: postpartum hemorrhage, neonatal resuscitation, and maternal seizure, practicing team roles and protocols. (6h)
  • Integrated Postpartum Care Simulation — Learners manage a comprehensive postpartum scenario from admission to discharge, including assessment, education, and referral, integrating all skills learned. (4h)

Summatives

Each delegate is assessed continuously throughout the course via daily exercises, scored practical assignments, and a final summative test at the end.

Practical Assignments — 30%

Practical assignments are observed and scored against a rubric during the practical sessions. Each delegate's practical mark is averaged into a single 100% score and contributes 30% to the final total.

Daily Exercises — 20%

Every training day ends with a multiple-choice exercise scored out of 100%. The scores from each daily exercise are averaged across the duration of the course to produce a Daily Average mark, which contributes 20% to the final total.

Final Test — 50%

On the last day a final summative test is written. It is a multiple-choice paper with multiple-answer questions: each question may have more than one correct option, and a single wrong selection on a question marks the entire question wrong — no partial credit. The final test is scored out of 100% and contributes 50% to the overall mark.

Certificate of Completion

Awarded to delegates who achieve an overall mark of 50% or higher on the Final Total (Practicals 30% + Daily Average 20% + Final Test 50%).
