This course equips learners with the advanced leadership and management skills required to effectively facilitate, lead, and manage Hazard and Operability (HAZOP) studies in line with South African and international best practices. It focuses on enhancing team performance, ensuring rigorous risk assessment, and driving a proactive safety culture within process industries.

Learning Outcomes

Target Audience

This course is designed for process safety managers, HAZOP team leaders, senior engineers, and health and safety professionals who are responsible for leading or managing HAZOP studies within their organizations.

Course Outline

Day 1: Foundations of HAZOP and Leadership

Objectives:
• Understand the purpose and principles of HAZOP studies.
• Identify the roles and responsibilities of a HAZOP leader.
• Recognise the importance of leadership in process safety.
• Apply relevant regulations and standards in South Africa.

Topics:
• Introduction to HAZOP: history, purpose, and benefits.
• HAZOP methodology: guide words, parameters, deviations, and consequences.
• The HAZOP team: composition, roles, and responsibilities.
• Leadership styles and their impact on team effectiveness.
• Overview of South African legislation (e.g., OHS Act, Major Hazard Installation regulations).
• Introduction to risk assessment and management principles.

Day 2: HAZOP Preparation and Team Dynamics

Objectives:
• Plan and prepare for a HAZOP study effectively.
• Manage team dynamics and facilitate productive discussions.
• Develop and use HAZOP documentation.
• Apply techniques for identifying credible scenarios.

Topics:
• Pre-HAZOP activities: scope definition, documentation review, and scheduling.
• Selecting and preparing the HAZOP team.
• Facilitation techniques: questioning, probing, and managing dominant personalities.
• Recording HAZOP findings: worksheets, action tracking.
• Handling deviations and guide words systematically.
• Common pitfalls in HAZOP and how to avoid them.

Day 3: Advanced HAZOP Techniques and Risk Assessment

Objectives:
• Apply advanced HAZOP techniques for complex systems.
• Integrate risk assessment methods with HAZOP.
• Evaluate risk and make decisions on acceptability.
• Understand the role of layers of protection in risk reduction.

Topics:
• Advanced HAZOP: batch processes, procedural HAZOP, and human factors.
• Linking HAZOP to LOPA (Layer of Protection Analysis).
• Risk matrices and risk criteria.
• Qualitative vs. quantitative risk assessment.
• Managing uncertainties and assumptions.
• Case studies of complex HAZOP applications.

Day 4: Leadership, Communication, and Decision-Making

Objectives:
• Develop leadership skills specific to HAZOP teams.
• Enhance communication and conflict resolution abilities.
• Make effective decisions under uncertainty.
• Manage time and resources efficiently.

Topics:
• Emotional intelligence in leadership.
• Communication styles and active listening.
• Conflict resolution and negotiation techniques.
• Decision-making models and tools.
• Time management and meeting facilitation.
• Building a positive safety culture.

Day 5: Practical Integration and Continuous Improvement

Objectives:
• Apply HAZOP leadership skills in a simulated environment.
• Evaluate HAZOP outcomes and drive continuous improvement.
• Develop an action plan for implementing HAZOP leadership in the workplace.
• Review and reflect on learning and personal development.

Topics:
• Full-scale HAZOP simulation exercise.
• Reviewing and closing out HAZOP actions.
• Measuring HAZOP effectiveness and KPIs.
• Continuous improvement in process safety management.
• Developing a personal leadership action plan.
• Course review and assessment.

Practicals

16 hours of practicals To be conducted online or on-campus or in-house
Overview

The practicals are essential for developing hands-on facilitation and leadership skills in HAZOP studies. Learners will engage in realistic simulations and exercises that reinforce theoretical knowledge and build confidence in leading HAZOP teams.

Practical Activities
  • HAZOP Preparation and Documentation — Learners work in groups to prepare for a HAZOP study on a given process, including defining scope, gathering documentation, and developing a HAZOP worksheet template. (4h)
  • Facilitated HAZOP Simulation — Learners lead a simulated HAZOP session with role-playing team members, guiding them through nodes, deviations, and cause-consequence analysis while managing group dynamics. (8h)
  • Risk Assessment and Decision-Making — Learners evaluate the risk of identified scenarios using risk matrices and decide on recommendations, considering layers of protection and cost-benefit analysis. (4h)

Summatives

Each delegate is assessed continuously throughout the course via daily exercises, scored practical assignments, and a final summative test at the end.

Practical Assignments — 30%

Practical assignments are observed and scored against a rubric during the practical sessions. Each delegate's practical mark is averaged into a single 100% score and contributes 30% to the final total.

Daily Exercises — 20%

Every training day ends with a multiple-choice exercise scored out of 100%. The scores from each daily exercise are averaged across the duration of the course to produce a Daily Average mark, which contributes 20% to the final total.

Final Test — 50%

On the last day a final summative test is written. It is a multiple-choice paper with multiple-answer questions: each question may have more than one correct option, and a single wrong selection on a question marks the entire question wrong — no partial credit. The final test is scored out of 100% and contributes 50% to the overall mark.

All marks are recorded on the AATICD LMS and visible to each learner under their account.

Certificate

Certificate of Completion

Awarded to delegates who achieve an overall mark of 50% or higher on the Final Total (Practicals 30% + Daily Average 20% + Final Test 50%).

  • If you scored under 50% you can sit the final test again at the next scheduled session at no extra cost.
