Description

This course equips learners with the knowledge and skills to safely handle, store, and dispose of medical waste in compliance with South African regulations. It covers classification, segregation, treatment, and disposal methods to minimize health and environmental risks.

Healthcare workers, waste management personnel, and environmental health practitioners involved in the generation, handling, or disposal of medical waste in clinical or laboratory settings.

Prerequisites

None — open enrollment. However, learners should be employed or involved in a healthcare or waste management environment.

Course Outline

Day 1: Introduction to Medical Waste and Regulatory Framework

Objectives:
• Define medical waste and its categories
• Understand the health and environmental risks of improper disposal
• Identify key South African legislation and regulations governing medical waste
• Explain the roles of SAQA, DoH, and DEA in waste management
• Describe the hierarchy of waste management (reduce, reuse, recycle, recover, dispose)

Topics:
• Types of medical waste: infectious, pathological, sharps, pharmaceutical, chemical, radioactive
• Sources of medical waste: hospitals, clinics, laboratories, veterinary practices
• Risks: infection, injury, environmental pollution
• Legal framework: National Environmental Management Act, Health Act, SANS 10248
• Waste management hierarchy
• Overview of waste management plan requirements

Day 2: Segregation, Packaging, and Labeling

Objectives:
• Apply correct segregation procedures for different waste types
• Select appropriate containers and packaging materials
• Label and color-code waste according to standards
• Implement safe handling practices to prevent spills and exposure
• Understand documentation requirements for waste tracking

Topics:
• Segregation at source: color-coded bins and bags
• Container specifications for sharps, fluids, and solid waste
• Labeling: biohazard symbols, waste category, date, source
• Handling procedures: PPE, no manual compaction
• Spill response and decontamination
• Waste manifest and chain of custody
• Auditing segregation compliance

Day 3: On-Site Storage, Collection, and Transport

Objectives:
• Design and manage on-site storage areas compliant with regulations
• Schedule and conduct safe internal collection rounds
• Prepare waste for external transport
• Complete transport documentation and placarding
• Inspect vehicles and containers for compliance

• Storage area requirements: ventilation, security, signage
• Temperature control for infectious waste
• Internal collection: routes, carts, schedules
• Transport containers: rigid, leak-proof, labeled
• Vehicle requirements: enclosed, washable, placarded
• Consignment notes and waste tracking
• Emergency procedures during transport

Day 4: Treatment and Disposal Technologies

Objectives:
• Compare treatment methods: autoclaving, incineration, chemical disinfection, microwave, irradiation
• Select appropriate technology based on waste type and volume
• Understand operational parameters for each method
• Monitor treatment efficacy (biological indicators)
• Manage residues and emissions responsibly

Topics:
• Autoclave: temperature, pressure, cycle time
• Incineration: types, combustion efficiency, ash disposal
• Chemical disinfection: contact time, concentration
• Alternative technologies: microwave, plasma, irradiation
• Validation and monitoring: spore tests, temperature logs
• Environmental controls: air emission standards
• Disposal of treated waste: landfill requirements

Day 5: Waste Management Planning, Auditing, and Continuous Improvement

Objectives:
• Develop a comprehensive medical waste management plan
• Conduct internal audits and inspections
• Identify non-conformances and implement corrective actions
• Train staff on waste management procedures
• Evaluate and improve waste minimization initiatives

Topics:
• Components of a waste management plan: policy, roles, procedures
• Risk assessment and contingency planning
• Audit checklists and reporting
• Corrective and preventive actions (CAPA)
• Training programs: induction, refresher, competency
• Waste minimization strategies: inventory control, reusable alternatives
• Record keeping and documentation review
• Case studies of best practices

Practicals

16 hours of practicals To be conducted online or on-campus or in-house
Overview

Learners apply theoretical knowledge through hands-on activities including waste segregation, packaging, handling, and treatment simulation. Practicals reinforce safe practices and regulatory compliance, ensuring learners can perform tasks competently.

Practical Activities
  • Segregation, Packaging, and Labeling Exercise — Learners practice sorting waste items into correct bins, packaging sharps and fluids, and applying labels with proper markings. (4h)
  • Spill Response and Decontamination Drill — Simulated spill of infectious waste; learners must contain, clean, and decontaminate the area using approved procedures. (3h)
  • Autoclave Operation and Monitoring — Learners load an autoclave, set parameters, run a cycle, and interpret biological indicator results to verify efficacy. (4h)
  • Internal Collection and Transport Simulation — Simulated collection rounds within a facility, using carts and containers, followed by loading onto a transport vehicle. (3h)
  • Waste Management Plan Audit Role-Play — Teams audit a mock waste management plan, identify gaps, and propose corrective actions. (2h)

Summatives

Each delegate is assessed continuously throughout the course via daily exercises, scored practical assignments, and a final summative test at the end.

Practical Assignments — 30%

Practical assignments are observed and scored against a rubric during the practical sessions. Each delegate's practical mark is averaged into a single 100% score and contributes 30% to the final total.

Daily Exercises — 20%

Every training day ends with a multiple-choice exercise scored out of 100%. The scores from each daily exercise are averaged across the duration of the course to produce a Daily Average mark, which contributes 20% to the final total.

Final Test — 50%

On the last day a final summative test is written. It is a multiple-choice paper with multiple-answer questions: each question may have more than one correct option, and a single wrong selection on a question marks the entire question wrong — no partial credit. The final test is scored out of 100% and contributes 50% to the overall mark.

Certificate of Completion

Awarded to delegates who achieve an overall mark of 50% or higher on the Final Total (Practicals 30% + Daily Average 20% + Final Test 50%).
